Program Case Manager Department: Men's Resident Ministries Reports
To: Sr. Director of Program FLSA Status: Regular, Full Time ,
Exempt SUMMARY: Demonstrate Christ-like behavior and attitude in
all job duties and responsibilities. The Case Manager must have
extensive experience working with individuals that are chronically
homeless, abusing alcohol/drug/addiction, domestic violence, human
trafficking victims, and recovery program services. The Case
Manager will provide overall program case management and
consultation to program participants and report to the Sr. Director
of Programs. ESSENTIAL DUTIES AND RESPONSIBILITIES Case management
activities involve bringing services, agencies, resources, and
people together within a planned framework of action toward the
achievement of established goals. It may involve liaison activities
and collateral contacts. This position involves all case manager
duties and guidelines, keeping confidentiality and HIPPA compliance
guidelines, and resident safety while performing and assisting with
the following duties: Case Management Monitors and evaluates the
efficiency and effectiveness of program service delivery, case
management methods, and procedures; recommends best practices for
computerized case management methods. Implements individualized
intervention and assistance in accessing public benefits/Medi-Cal
Outreach and Enrollment; information/referral regarding access to
health; and encourages participation in educational and
career-related opportunities. Provides comprehensive case
management across all mission-wide programs and aids clients on
caseload to employ interventions to resolve problems and evaluate
progress using the SMART plan approach. Utilizes evidence-based
best practices, intensive case management, behavioral contracts,
critical time intervention, motivational interviewing, and
trauma-informed approaches in the coordination of program services;
including but not limited to the following: bio-psychosocial health
assessments, progress checks, verifications/completion letters,
homelessness certifications, goal setting, and treatment planning.
Coordinates/monitors and works with the court system, DOJ, DMH, and
social services to coordinate program services in alignment with
court orders, probation department, and social services mandates.
Provides ongoing monitoring of client’s well-being, linkages to
mental health services, linkages to permanent housing, and other
support services as needed. Conducts crisis intervention that
assists/supports the client’s ability to independently
problem-solve by utilizing problem-solving skills and manager
selfcare. Provides crisis intervention and resolve conflicts
between staff, participants, and participants’ families as needed.
May be required to work some evening hours and/or weekends as an
exempt status employee as needed. Record Keeping Maintains copies
of provider literature and program materials for participants to
review. Manages required client charting and documentation in a
timely and thorough manner. Serve as the liaison and gatekeeper for
the administration of the Client Track system ensuring that the
client data is kept up to date and all relevant data is recorded in
the Client Track system. Works directly with the Intake coordinator
to ensure completion and quality control of all resident files and
updated information in the Client Track system . Produce regular
Client Track reports on the number of participants enrolled,
served, and discharged, conduct daily/monthly tracking/monthly
reporting, and track occupancy fees, SSI, GR, food stamps,
gratuity, and other cost analysis reporting. Community &
Interdisciplinary Collaboration Develops and maintains proactive
working relationships within the mission, community, organizations,
and councils to expand program awareness, referrals, and services.
Works collaboratively with participants to develop
short-term/long-term goals and decides program services needed,
monitors client progress and ensures treatment plan outcomes are
met and/or changed in collaboration with the client and the
interdisciplinary support team as appropriate. Attends weekly team
case managers meetings and scheduled interdisciplinary team
consultations to supply feedback on the status of the participant
and collaboratively work to ensure the participant’s continued
progress. Collaborates with counselors, Interns, or mental health
counselors to ensure that each participant’s transition plan is
consistent with their individualized treatment and rehabilitation
plan. Works with the interdisciplinary team through the entry
pathway from the application/ intake/Information gathering process
to discharge/exit planning and confirms a referral process to
ensure continued stabilization, higher level of care, and
successful completion of goals, employment, and housing services.
Facilitates weekly groups and co-facilitates with other certified
SUD counselors as assigned throughout the year. Communicates with
network providers as participants move between levels of care and
into post-discharge recovery services to support successful
transition(s). Creates and develops relationships with community
providers of housing, employment, education, food assistance,
childcare support, substance abuse treatment, primary care, mental
health treatment, and other supportive services that will foster
reentry and connectivity to the community at large. Chaplaincy
Accountability Offers prayer and encouragement consistent with
scripture and the principles of Los Angeles Mission, Inc., as
deemed appropriate. Participates in chaplaincy-related outreach,
presentations to partnering churches, special events, Mission’s
family devotion, and other community relations at the Los Angeles
Mission, Inc., upon request of Director of Ministry and
notification of Program Manager, in tandem with responsible
caseload management. Engages in assigned rotation to teach
delegated learning center classes. Attends voluntary weekly staff
prayer, prayer walks, and other Ministry duties that do not
interfere with caseload management. SUPERVISORY RESPONSIBILITIES:
