Travel Assignment) Position Overview Cone Health is seeking a
skilled Oncology Case Manager Registered Nurse for a 13-week travel
assignment in Yorba Linda, CA. This role focuses on coordinating
care for oncology patients and associated diagnoses, ensuring
quality outcomes and continuity of care across the patient journey.
Schedule & Location - Shift: Monday–Friday, 5x8-hour days -
Location: Yorba Linda, CA 92886 - Start Date: December 29, 2025 -
Duration: 13 weeks Responsibilities - Provide case management
services for oncology patients and related diagnoses. - Collaborate
with interdisciplinary teams to develop, implement, and evaluate
individualized care plans. - Ensure compliance with clinical
standards, regulatory requirements, and organizational policies. -
Support patients and families through education, advocacy, and
coordination of resources. - Utilize Epic charting system to
document and track patient care. Required Qualifications - Minimum
1 year of case management experience. - Minimum 1 year of oncology
nursing experience. - Current BLS certification. - ONS Chemotherapy
Certification. Preferred Qualifications - Ambulatory care
experience. - Strong communication and organizational skills. -
Ability to work independently and collaboratively in a fast-paced
environment. Certifications - Required: BLS, ONS Chemotherapy
