Key Responsibilities
Provide telephonic patient care and support for medication
adherence and disease management
Collaborate with healthcare professionals to develop and evaluate
comprehensive care plans
Utilize telecommunications nursing outreach programs to support
defined patient populations
Required Qualifications
Registered Nurse with an unrestricted current compact license in
Texas
3+ years of clinical RN experience
Experience using Microsoft Office, including Word, Excel, and
Outlook
Candidate must be based in Texas
RN Diploma, Associates, or Bachelors in Nursing
